on the app in the top right corner there is a + button, and you can add a photo or video to the pattern you want!

If you're using the website, you click onto the pattern you want to upload to and in the top right corned there is an add photo button

I recommend first taking a photo the way you would usually take a photo and then submitting it to the page like described above.

THe photo will also need to be approved by a moderator before going on the site to make sure the photo is good(not blurry, good lighting, correct pattern etc.)
