There shouldn't be holes, but holes can happen maybe you forgot a knot or did leave too much loose string between knots. Holes can also happen when making a colour switch when not using the flat alpha technique.

You can also try to help this problem by pulling the first half of the knot tighter then finishing it when the knot before it is as close as you want it to be.
